You can use this application to apply for the determination of a disability and the degree of disability.
If you meet the requirements, a degree of disability (GdB) will be determined. The GdB is between 20 and 100 depending on the severity of the impairment. If you have a GdB of 50 or more, you can be issued with a severely disabled person's pass.
In addition to the degree of disability, certain health restrictions can also result in the identification of special features. These characteristics are entered in the severely disabled person's pass and entitle you to further compensation for disadvantages.
You can apply for the following marks:
G - significant impairment of mobility in road traffic
aG - exceptional walking disability
H - helplessness
B - entitlement to be accompanied at all times when using public transport
RF - Reduced radio license fee and/or reduced telephone connection fee
GL - deafness
BL - Blindness
TBL - Deafblindness (exemption from broadcasting fees)

- if applicable, power of attorney, proof of guardianship, proof of custody
- for nationals of a non-EU member state: copy of the residence permit
- if you live abroad and work in Germany: certificate from your employer
Optional:
- Documents about your state of health (e.g. doctor's letters, hospital reports, rehabilitation reports, medical reports)
- Notifications from other bodies regarding a reduction in earning capacity (MdE) or a degree of impairment (GdS)
When assessing the degree of severity of the GdB to be determined, only those health restrictions are taken into account which have existed for more than six months and are proven by current reports or expert opinions.
The district or city in which you live is responsible for your application. You will receive a confirmation of receipt from them with further information.
Your application and the documents you have submitted will be checked. If necessary, the responsible office will independently request further documents from doctors or institutions in order to clarify the medical facts sufficiently.
Your documents will be evaluated with the involvement of a doctor. You will receive a notification from the responsible office about the degree of disability (GdB) and any special features.
If the GdB is 50 or more, you can also receive a severely disabled person's pass in cheque card format after submitting a photograph. This will be sent to you separately via a service provider.
You can submit an application under the law on severely disabled persons at any time. Although an informal application is possible, you will need to complete the application form afterwards.
You must give your written consent for your data to be passed on to the service provider before an ID card can be issued.
In order to claim the disability lump sum from the tax authorities, you must provide your tax ID and consent to the transfer of data.
A release from confidentiality signed by you is required for further clarification of the facts.
Your obligation to cooperate in the procedure (obligation) arises from § 60 Para. 1 of the First Book of the German Social Code (SGB I). If you do not comply with this obligation, the determination in accordance with SGB IX may be refused in whole or in part if the requirements are not met.

- Medical documents are requested by the competent authority from doctors, hospitals, rehabilitation facilities, etc. and also from the pension insurance, nursing care insurance, employers' liability insurance association, employment agency, etc.
- if an examination is required, the applicant will be informed accordingly; there is no entitlement to an expert examination
- a medical opinion is issued independently by the competent body after the facts of the case have been clarified
- on the basis of the medical opinion, the competent authority issues a decision on the degree of disability (GdB) and any characteristics or a decision rejecting the determination of a disability
- Competent authorities in North Rhine-Westphalia: district or district-free city according to place of residence
