You can apply for a grant for your fertility treatment (in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) treatment).

Purpose

Funding is available for the costs of in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) treatments for the first to fourth treatment cycle. The aim is to make it easier for couples with an unfulfilled desire to have children to access reproductive medicine.

Object

Couples receive a subsidy towards the costs of fertility treatment, depending on their remaining treatment costs after settlement with their health insurance provider.

Beneficiaries

Beneficiaries are heterosexual married couples or heterosexual couples living in a non-marital partnership. Both partners must have reached the age of 25. The woman must be under the age of 40 and the man under the age of 50.

The couple must have their joint main residence in Bavaria.

Couples who are not married must be in a long-term partnership. A long-term cohabitation does not permit any further cohabitation and is characterized by an inner bond. It can be assumed if - according to the declaration of the unmarried couple - they live together in a stable partnership and the unmarried man declares that he will recognize the paternity of the child conceived through reproductive medicine.

Type and amount

The allowance for married couples is up to 50 percent of the remaining personal contribution for all four treatment cycles, for couples in a non-marital partnership up to 25 percent for the first to third treatment cycle and up to 50 percent of the remaining personal contribution for the fourth treatment cycle.

The maximum amounts for all couples are EUR 800 for IVF treatment in the first to third treatment cycle and EUR 1,600 in the fourth treatment cycle, and EUR 900 for ICSI treatment in the first to third treatment cycle and EUR 1,800 in the fourth treatment cycle.

Erforderliche Unterlagen

  • Please submit the following attachments with your application:

    For applicants who are married to each other:

    • From all spouses:
      • Copy of marriage certificate
      • ZBFS form "Medical certificate of necessity".
    • From those with public health insurance in addition:
      • For the first to third treatment cycle:
        • the treatment plan approved by the statutory health insurance with the expected treatment costs to be incurred
        • Covering letter of the health insurance company of the wife to the treatment plan
        • Accompanying letter from the husband's health insurance for the treatment plan
      • For the fourth treatment cycle:
        • Treatment plan with the expected costs (cost estimate)
    • From privately insured persons, persons entitled to benefits/assistance and persons entitled to other benefits additionally:
      • Treatment plan with the expected costs (cost estimate)
      • Declarations of cost assumption or negative decision

    For applicants living in a non-marital partnership

    • From all applicants:
      • Treatment plan with the expected costs (cost estimate).
      • ZBFS form "Medical certificate on the necessity".
    • From privately insured persons with health insurance, persons entitled to benefits/assistance and persons entitled to claim from other benefit providers in addition:
      • Declarations of cost assumption or negative decision

Voraussetzungen

Further requirements in addition to those listed under "Recipients" are:

  • A medical consultation on the medical and psychosocial aspects of fertility treatment must have taken place. This consultation will usually be provided by the specialist treating you before you receive a referral to a fertility clinic. Please note that the consulting doctor is not allowed to carry out the fertility treatment themselves.
  • Only the couple's own egg and sperm cells are used for the treatment.
  • A medical declaration (certificate) from your specialist (e.g. gynecologist) or the fertility clinic must be available stating that the fertility treatment is necessary and that there is a reasonable chance that the treatment will result in pregnancy.
  • The fertility treatment must be carried out in a clinic or by a doctor based in Bavaria or a neighboring federal state (Baden-Württemberg, Hesse, Thuringia, Saxony). In the case of couples for whom an applicant has a claim for the specific treatment cycle against their statutory health insurance, the treatment must be carried out in a clinic or practice with approval in accordance with §121a SGBV.
  • The treatment has not yet begun: Prescriptions for the treatment cycle for which funding is requested may only be redeemed once you have received the grant notification.

Verfahrensablauf

  1. Medical advice on the treatment, in particular on the medical and psychosocial aspects, from a doctor who does not carry out the treatment themselves.
  2. Obtaining a medical declaration on the necessity and prospects of success of the treatment in accordance with Section 27 a (1) Nos. 1 and 2 SGB V (not required if the treatment plan is approved by the statutory health insurance provider).
  3. Preparation of a treatment plan including a cost breakdown or a cost plan by the attending physician at the pediatric clinic.
  4. Obtaining approval or rejection of the treatment plan from statutory health insurers or obtaining declarations of assumption of costs or rejection thereof from private health insurers and, if applicable, from aid/assistance offices or another service provider (supplementary insurance for fertility treatment).
  5. Apply to the Zentrum Bayern Familie und Soziales (ZBFS) for approval of the subsidy.
  6. Wait for the grant notification from the ZBFS.
  7. Only then: Fill prescriptions or purchase medication required for fertility treatment.
  8. Submit invoices and receipts for treatment costs to health insurance companies and, if applicable, to benefits offices/assistance offices or other service providers (if the assumption of costs has been declared).
  9. Submit an application for payment of the subsidy to the ZBFS with copies of
    • all invoices from the reproduction facility, pharmacy, laboratory and anesthesia (if applicable) and
    • Proof of reimbursement from all health insurance companies involved.
  10. The treatment and its settlement with the health insurance companies must be completed within the 8-month approval period specified in the grant notification. The application for disbursement with the attached documents must be received by the ZBFS no later than the last day of the approval period. Otherwise no payment can be made.
  11. The grant will be paid out after the application for payment has been checked by the ZBFS.

Bearbeitungsdauer

It takes around three to four weeks from receipt of your funding application to receipt of the grant notification. If additional documents need to be requested, it may take longer. Please take this into account when planning the timing of your fertility treatment.

Hinweise

Please note that the measure is only eligible for funding if the treatment of the respective eligible treatment cycle, including drug treatment, has not yet begun. The purchase of medication or the filling of prescriptions required for fertility treatment counts as the start of the measure. This may only take place after receipt of the grant notification.

The preparation of the treatment plan and the declaration of assumption of costs by the statutory health insurance fund, the subsidy or the private health insurance fund are not considered to be the start of treatment for the purposes of these guidelines.

A separate grant must be applied for each treatment cycle and, if applicable, each repeat attempt.

After the birth of a child, the treatment cycles are again eligible for funding if the pregnancy leading to the birth of the child occurred within the four eligible treatment cycles.
