The service with the combination form can be used for births in Hamburg maternity clinics, in the Hamburg birth center or in the house for birth and health. The form is available online. Parents can also obtain a paper copy from the birthing facility.

You can use the service for births in all Hamburg maternity clinics and in the Hamburg birth center.

- receive the tax identification number for your child
- and apply for child benefit at the same time.

You can use the service for births in all Hamburg maternity clinics and in the Hamburg birth center.
Unless otherwise stated, please enclose the originals of the following documents. Foreign-language documents must be submitted with a translation into German, which was prepared by a publicly sworn or recognized translator. Your originals will be returned to you along with your child's birth certificates.
- For married couples: marriage certificate with reference to the birth certificate of the spouses (if there is no reference, the birth certificates of the spouses must also be submitted); if necessary, "Certified copy of the family book" (if you married before 2009).
- For unmarried couples: mother's birth certificate. For the registration of the father: the father's birth certificate and the certificate of acknowledgment of paternity. If the parents have joint custody, also enclose the certificate of custody. If available, also present the certificate of naming.
- If the mother is divorced or widowed: marriage certificate stating the dissolution of the marriage. If the information is missing, the divorce decree or the death certificate must also be submitted.
The registry office can request the submission of further documents if this is necessary to prove information.
You can reach the registry offices at the address given on the information sheet for your birth facility and in the “Information on service” on page 4. The contact details of the Family Benefits Office can be found under Links.
The service with the combination form is available to all parents who give birth to their child in a Hamburg maternity clinic, in the Hamburg birth center or in the house for birth and health. You do not have to be registered in Hamburg.
Parents receive birth certificates for parental allowance and maternity assistance free of charge.
In addition, other fee-based German and/or international birth certificates for personal use can be ordered with the combination form (e.g. for kindergarten, baptism, application for an ID card/passport, etc.). The amount of the fees is printed on the envelope that parents receive in their birth center or on the "Information sheet of the Hamburg registry offices".
The parents pay for the additional birth certificates without cash by transferring the total amount to the account specified on the information sheet for the birthing facility and on page 3 of the "Service information".

- The combination form with the associated instructions and information is available online for you to fill out and print out - see links. If necessary, you can also get it in paper form from your Hamburg maternity clinic or in the Hamburg birth center.
- You fill out the combined form completely, sign the second and third page, enclose your certificates and put everything in the envelope that you will receive at your birth facility. You can return the filled envelope there after the birth of your child.
- You transfer the fees for the birth certificates you have ordered to the account specified on the information sheet for your birth facility and on page 3 of the “Service information” immediately when submitting your documents.
- The birth center hands over the envelope to the responsible registry office in Hamburg.
- In addition, it notifies the registry office of the birth of your child.
- The registry office certifies the birth of your child and creates the desired birth certificates for you, which you will receive by post.
- The registry office will then immediately forward your child benefit application to the family benefits office. You no longer have to submit your child's birth certificate to the Family Benefits Office.
- At the same time, the residents' registration office receives notification of the birth of your child from the registry office and transfers the data to the population register. In addition, information for statistical purposes is forwarded to the Statistical Office for Hamburg and Schleswig-Holstein.
- The Federal Central Tax Office (BZSt) receives the data about your child from the residents' registration office in order to then create a tax identification number for your child, which you will receive by post.
- The BZSt transmits the tax identification number required for the child benefit application to the Family Benefits Office.
- When the Family Benefits Office has received the tax identification number, your child benefit application will be notified by post.
After about 10 days, you will receive the requested services by post, provided that all the necessary documents for the certification of the birth and the notification of the child benefit application are available. If something is missing, the institution asks for support using the contact information provided.
